I, the parent of the above listed participant, request voluntary participation for the registered child in my custody to participate in The RUSH and all that it entails which are hereinafter referred to as the “activity”.
Knowing and understanding the risks involved with participation in the activity, I hereby voluntarily and willingly assume responsibility for all risks and dangers associated with the registered child’s participation in the activity. I agree I am financially responsible for any losses resulting from their actions and will indemnify Hope Community Church and their employees and agents of each of them, for any loss or damage caused by myself or my registered child’s during this activity.
In consideration of my and my registered child’s participation in the activity, I hereby waive all claims or causes of action against Hope Community Church and the employees and agents of all of them, arising out of participation in the activity and hereby release, hold harmless, and discharge Hope Community Church and their employees and agents and each of them from all liability in connection therewith except such loss or damage which was caused by the gross negligence or willful misconduct of Hope Community Church and their employees and agents.
I have read this waiver of liability agreement and understand the terms used in it and their legal significance. This waiver and release is freely and voluntarily given with the understanding that right to legal recourse against Hope Community Church and their employees and agents of is knowingly given up in return for allowing participation in the activity.
